How can I connect Headphones to a telephone handset?

I want to connect ordinary PC stereo (or mono) headphones with Mic to an Avaya 4610SW IP phone. The headphones with mic set has two points for connecting to PCs, while the telephone handset has a telephone jack point. I am open to some crimping/ joining if anyone can give me some detailed wiring diagrams.
