When did Lucent 2 for 1 stock splits occur?

I'm doing my taxes and I need a basis for Lucent stock up until 2000 and I can't remember when the stock split. I believe it split twice and I know one time was in 1998 but I can't seem to find out when the 2nd split happened. I need to know what the Lucent stock cost so I can figure out what the Avaya stock cost basis should be.
